Khalid, a 33-year-old man presents to your clinic, complaining of generalized abdominal pain.
Abdominal examination is normal

Scenario
- You are a 33-year-old man, a teacher, married with 2 children, and have come complaining of generalized abdominal pain.
Opening Statement
“Doctor, I keep on having this pain in my tummy” (Point towards your abdomen)
Information to be given in response to specific questions
This pain, which is all over your tummy, started 8 months ago – give this information in response to the first query by the student.
- You have this pain on 6-8 days per month; it comes and goes.
- Associated with alternating bowel habits (diarrhea and constipation).
- When it is diarrhea, you pass 3-4 loose stools per day after meals.
- When it is constipation, you pass a hard stool every 3rd day.
- Increased by stressors at home and at work and by milk ingestion.
- No loss of appetite and no weight loss.
- No per rectal bleeding, no nausea or vomiting.
- No history of colon cancer in your family or other GI problems in your family.
- No fever.
- No history of any GIT infection before the start of these symptoms.
- No history of physical or sexual abuse as a child.
- No symptoms of depression, e.g., low mood, loss of interest, sleep disturbance, and excessive tiredness, etc.
Say no to any other complaints not included in this scenario, if asked by the student.
So far, you have coped without any medication, but now you need advice and help to cope with this problem.
- You don’t have any idea or concern about the pain, but you expect good treatment for the pain as it is disturbing your performance at work.
- Past history: not significant.
- Medication: nil.
Family History
- Not significant.
Social History
- Married, you have a 4-year-old daughter and a 2-year-old son.
- You are a teacher, non-smoker, and don’t take alcohol, but you get a little tense while coping with work at the school and at home.
- There is no conflict at home or at work.
Dress and Behavior
- Casual dress, but you look a little anxious.
