Predisposing Factors

Causative Agents:

Pneumococci (80%) Typical hypopyon ulcerZ

  • Morax Axenfield Bacillus (20%)
  • Streptococci, Staphylococci, Pseudomonas and Fungi

Clinical Picture (the same)

Symptoms

  • Pain
  • Photophobia
  • Lacrimation
  • Blepharospasm
  • Poor vision

Signs (Acute Serpiginous Ulcer)

  1. Ciliary injection

  2. Haziness of the cornea (loss of luster)

  3. Ulcer Near the centreZ

    • Central advancing Edge
    • Peripheral Healing Edge
  4. Fluorescein Test is +ve

  5. Hypopyon in the Anterior Chamber

    • (Steril Pus) PNL +Fibrin +Iris Pigment

NB Perforation is common…why?

Treatment of Hypopyon Ulcer

  • Treatment of the cause (Dacryocystectomy)

  • Usual Treatment of corneal ulcer ABCD

    • Fortified Eye Drops

      • Gentamycin or Tobramycine 15mg/ml
    • Subconjunctival Injection of AB

      • Cephazoline (100mg in 0.5 ml)
      • Tobramycin or Amikacine (20mg in 0.5 ml)
    • Treatment Of complication : 2ry Glaucoma
