Knee Clinical Examination Station

The task involves examining a patient with pain in the right knee.

Examination Structure

The examination is structured into several key components: Opening, Look, Feel, Move, and Special Tests. Each section has specific procedures and weightage for assessment.

Opening

  • Introduction, Privacy, Permission & Exposure: 2 marks
  • Deformity and Extra: 1 mark

Look

  • Inspect for deformity, skin, muscle bulk, and bony landmarks: 1 mark
  • Examination of the back of the knee: 1 mark

Feel

  • Assess tenderness (general and localized):
    • Quadriceps, Patella, Patellar tendon, Tibial tuberosity, Origin, and insertion of collateral ligaments: 2 marks
  • Joint-line tenderness (medial and lateral): 1 mark
  • Temperature comparison (right vs. left, distal vs. proximal): 1 mark
  • Examination of the back of the knee for baker’s cyst and tenderness: 1 mark

Move

  • Active movement first for general screening of pain, followed by passive movement: 1 mark
  • Assess Flexion & Extension: 1 mark
  • Check for Effusion using either Milking OR Patellar Tap: 2 marks

Special Tests

  • Assess Collateral Ligaments using Varus and Valgus stress at 30 degrees: 2 marks
  • Evaluate ACL & PCL using Lechemann OR Drawer test OR Pivot Shift test: 1 mark
  • Assess PCL using Drawer test OR posterior Sag test: 1 mark
  • Evaluate Meniscus using McMurry’s test OR Medial & Lateral joint tenderness: 2 marks
  • Perform Patellar test using Patellar Tracking OR Patellar Grinding: 1 mark

Overall Assessment

  • Attitude with the patient: 1 mark
  • Total Marks: 22
