Congenital Subglottic Stenosis

  • Definition: Subglottic area is the narrowest area in the airway; stenosis if the diameter is less than 4 mm in term infants.

Symptoms

  • Biphasic stridor
  • Recurrent croup

Diagnosis

  • Bronchoscopy,
  • plain X-ray,
  • HKV X-ray

Airway Obstruction Grading

  • Grade I: <50% obstruction
  • Grade II: 51-70% obstruction
  • Grade III: 71-99% obstruction
  • Grade IV: No detectable lumen

Treatment

  • Grade I & II: Observation, balloon dilation, laser excision
  • Grade III & IV: Tracheostomy, laryngotracheal reconstruction, cricotracheal resection
